13. MANAGING CLIENTS WITH ELEVATED SUICIDE RISK
SUICIDE ASSESSMENTS
Working with a client at increased risk of suicidality requires empathy, regular assessment, and continual awareness of their risk and protective factors. The Life Stone Suicide Risk Assessment is derived from the Columbia Suicide Severity Rating Scale (C-SSRS) and included in client's intake paperwork. There are also physical copies available on the Life Stone Library, HERE.

SAFETY PLANS
When safety planning with clients, clinicians may use the standard Life Stone Safety Plan on the Shared Clinical Resource Drive HERE or one of their own choice.

TREATMENT NOTES FOR SUICIDAL CLIENTS 
Should you have a client struggling with suicidal ideation (SI) in session, be sure to document this in your therapy note. Clinicians should notate any information concerning: active vs. passive SI, safety planning, resources offered to the client, and any plans you made together. Be sure to upload any safety plans to eCharts.  

ACTIVE  SUICIDALITY IN SESSION
If you deem your client is an imminent threat to themselves and hospitalization is required, use the appropriate emergency code (purple) and procedure.

CLIENT HOSPITALIZATION
If a client has gone to the hospital or if you sent the client to the hospital for suicidality, please make other Life Stone clinicians and/or medical staff who work with the client aware.
