LICENSING RULES
Life Stone is committed to full compliance with all licensing requirements established by the Utah Department of Health & Human Services. Below is a summary of both the general licensing provisions and the rules specific to outpatient treatment programs for your review.
All of these requirements are addressed in Life Stone’s Policy & Procedures Manual. Additional guidance on these topics may also be found in the Employee Manual or Staff Reference Guide.
BACKGROUND CHECKS
All Life Stone staff are required to complete a background screening as a condition of employment. Background checks are conducted through the State of Utah’s DACS program and include Rap Back monitoring, which provides ongoing notification of any new criminal history information.
New employee’s awaiting background screening clearance must be supervised until they are determined eligible.
Staff are responsible for maintaining eligibility for employment and must promptly disclose any criminal charges, arrests, or convictions that may impact their ability to perform their role or meet licensing, ethical, or regulatory requirements.
In the event that a Rap Back notification is received, Life Stone management will review the information in a timely and confidential manner. Appropriate action will be taken based on the nature of the information, applicable laws and regulations, licensing requirements, and organizational policy. Actions may include further review, temporary restrictions, or employment-related decisions as deemed necessary.
All background check records and Rap Back notifications are securely maintained in compliance with state and regulatory requirements.
