Pre-Register from Home
You can pre-register prior to your first visit with us. Many clients find this more convenient than completing our new client forms in our lobby.
Refund Policy Life Stone requires payment at the time of services rendered. All programs require prepayment in full before a client starts his or her treatment at Life Stone. If you have insurance Life Stone will bill the clients insurance company, a prepaid co-pay will be collected before all services are rendered. Clients who stop treatment will not be granted a refund. Refunds will be granted for clerical and accounting errors. If you request a refund due to accounting errors this must be sent in writing to Life Stone with in 30 days with written proof of the error. |
